Month 02 — Mortality & Medicine

Core distinction

Disease — what the scan and pathology describe.

Illness — what the patient is actually living: fear, loss, identity, family strain (Kleinman).

Skull base disease routes vision, hearing, face, balance, independence through centimeters of bone — identity is the real stakes, not just survival.

Key questions per text

Tolstoy: Where am I at risk of becoming Ivan Ilyich — successful by every visible metric, quietly absent from the one life I have?
Gawande: Am I optimizing for the chart's version of success, or for what this person needs to still be able to do?
Charon: Am I letting the patient narrate the illness, or only interviewing them for disease data?

Three questions to have ready in clinic (Kleinman/Charon): "What are you most afraid this tumor will take from you?" · "What would a good outcome mean in your life?" · "What do you need to be able to do again?"

The month practice

After clinic, once a week: write one patient's illness story — not the disease summary.

Pick one patient from that day who stayed with you.
150–300 words, first person, from their vantage — what the disease threatens in their life.
No diagnosis codes, staging, or plan language — rewrite if you catch it.
End with: what a good outcome would mean to them.
Save to learning-records/illness-stories/YYYY-MM-DD-short-id.md — no identifiers.

Sob medida — your ledger risk

The pattern: achievement-fused identity — worth measured in output; the standard rises with the result, so satisfaction never arrives (hedonic treadmill).

The counter-move: name presence as done, out loud, the same nights you'd log cases and word counts — don't wait for a bigger milestone to feel it.

The tell: a 72-hour week of real output that still "feels unproductive," or a free evening spent getting ahead instead of counted as itself.

Maxim

Treat the tumor. Attend to the person losing something to it.

Second maxim — the ledger

A week counted only in output is a week Ivan Ilyich would recognize as his own.
